In this episode, we will discuss the growing fragmentation of the health care system, and what we can do about it by coordinating care. Nurses are tasked with discharge planning, case management, utilization review, quality assurance, patient/family education, and interprofessional collaboration. Having an effective care coordination plan in place can effectively decrease readmission rates.  Let's dive into how we can better support our medical staff in their roles while helping patients navigate the maze of specialists, procedures, medications, and follow-up appointments.
